Background
In cholesterol homeostasis regulation, the low density lipoprotein receptor is a critical factor. LDLR binds and internalizes LDL, then directs LDL toward cellular synthetic machinery, or towards destructive pathways for release or recycling. When alternatively spliced forms of the LDLR are present in cells, its native enzymatic activity is often reduced. This reduction in enzyme function is often related to the formation of atherosclerotic plaques. A common Class 5 mutation in LDLR results in it being able to bind and internalize LDL, but subsequently unable to release it to endosomes for recycling. LDLR is dependent upon the activity of PCSK9 for regulating its activity. In the liver, the LDLR is degraded when signaled by PCSK9, which increases the probability of high hepatic levels of LDL. In the brain, PCSK9 downregulates LDLR levels during development and following adult brain injury, thus demonstrating the dual functionality of this critical receptor.
